AMH GROUP SERVICE TICKET SUBMISSION FORM
AMH GROUP customers are able to submit service ticket requests using the online form below. Once submitted, an AMH GROUP represenative will be in contact with you regarding your request. It is our goal to complete most service requests within 2 business days of submission, when possible.
All service tickets are considered open (incomplete) until a completion of request notice AND a completion code are sent to the requesting customer. An incomplete service ticket means any discussed changes to your insurnce policy have not been finalized with your insurance carrier. The completion of request notice AND a completion code are our way of letting customers know their request has been complete, and their policy has been updated/implemented.
