Need to Update Info On A Policy?

Whether it is time for your policy to renew or you just need to update information on your policy, use the form below to make sure we have the most up-to-date information. We created a checklist to help you identify when to use the form.

Possible Personal Changes

  • Update email, phone number, etc

  • Change in job or starting a business

  • Buying a home/moving in the near future

  • Expecting a new child or parent moving in

  • Number of vehicles in house, drivers in house,

  • New purchases - boats, motorcycles, airplanes,

  • New property - rental property, condo

  • Items to be insured - jewelry, golf clubs, firearms, furs, etc.

  • Review your personal policies with AMH Group or elsewhere

Service Ticket Submission Form

AMH GROUP clients are able to submit service ticket requests using the online form below.  Once submitted, an AMH GROUP represenative will be in contact with you regarding your request.  It is our goal to complete most service requests within 2 business days of submission, when possible.

 

All service tickets are considered open (incomplete) until a completion of request notice AND a completion code are sent to the requesting customer.  An incomplete service ticket means any discussed changes to your insurnce policy have not been finalized with your insurance carrier. 

 

The completion of request notice AND a completion code are our way of letting clients know their request has been complete, and their policy has been updated/implemented.   

NOTE: Please note, BUSINESS INSURANCE requests include workers comp, cyber liability, general liability, employee benefits, commercial auto/property, bonding, professional liability insurance, D&O, and more.
